Waiver Request for Community Participants

Complete the BYU–Hawaii Waiver Request for Community Participants form found below at least 21 business days prior to the date of your event. Please provide as much detailed information and fill out form completely for a timely process. If there are any questions, you will be contacted.

Risk Management will determine if a waiver is required based on the details submitted.

Submit a copy of all signed waivers to University Scheduling.

If minors are participating you must complete the following:

3. All adult students/staff assisting with event must complete the Protection of Minor training. Put the name of your event in the "Youth Affiliation" field.

Approval Agreement
I understand that this event must support and further the mission and purpose of Brigham Young University–Hawaii, the Faculty, and/or department and that my Dean or Director as well as my direct upline Vice President must approve this event prior to scheduling. Academic buildings are intended to be scheduled only for academic purposes.
