Elementary and Secondary School Visits to BYU–Hawaii
Is your department having a campus tour for an elementary or secondary school? Are you putting on a concert or play for a elementary or secondary schools to attend on campus? Has an elementary or secondary school contacted you to do a job shadowing on campus?
If so, following these easy steps will help you have a successful event.
- Schedule the space.
- Download the appropriate form and have the school complete and return it at least 15 business days in advance of the event. Send a copy of the completed form to University Scheduling at universityscheduling@byuh.edu and keep a copy for your department.
- Hawaii Public (DOE) Schools
- Use the Hawaii DOE Authorized Educational Visit Agreement.
- DOE school will need to submit the Statement of Self-Insurance and Responsibility of the State of Hawaii letter from the State of Hawaii's Department of Accounting and General Services. Send a copy to University Scheduling at universityscheduling@byuh.edu and keep a copy for your department.
- All other elementary or secondary schools
- Use the Authorized Educational Visit Agreement - Non DOE Schools.
- School will need to submit a Certificate of Liability of Insurance along with endorsements as outlined in the agreement.
- Hawaii Public (DOE) Schools
- ALL BYU–Hawaii faculty, staff, student workers, service missionaries, etc. involved in the event is required to complete the Protection of Minor training steps one and two.
- Provide a list of all involved to University Scheduling at least 10 days prior to the day of your event with the:
- First and last name.
- BYUH ID #.
- When completing the Youth Protection Acknowledgement: put the Name of Your Event with the Date of the Event in the "YOUTH AFFILIATION(S)" field.
