Sponsorship of Non-BYUH Organizations

This sponsorship application form is required for ALL BYU–Hawaii groups wishing to bring non-BYU–Hawaii groups to campus for activities/meetings involving anyone outside of the sponsoring organization. Additional Indemnification and Liability coverage may be required. The completed sponsorship application (cover sheet and attached documentation) must be submitted a minimum of 21 working days prior to the activity/program. Full sponsorship authorization is required before facility or equipment usage can be confirmed. Such events are subject to established scheduling priorities and fees.

In addition, the BYU–Hawaii Waiver Request for Community Participants form must be completed. Risk Management will determine if a waiver is required based on the details provided.

If Minors Are Participating:

The Protection of Minors Training must be completed by ALL BYU–Hawaii adult faculty/staff/students participating in the event. You are required to submit a list of the adult staff to University Scheduling at universityscheduling@byuh.edu. Those who have not completed the training WILL NOT be able to participate.

Return the completed paperwork to the University Scheduling office in the Lorenzo Snow Administration Building, Room 263, or via email at universityscheduling@byuh.edu for final approval.
